Panduan Pemakanan

PANDUAN PEMAKANAN BAGI PESAKIT KOLESTEROL TINGGI

Ambil sumber haiwan daripada jenis ikan, ayam tanpa kulit atau arnab. Gunakan susu atau yogurt rendah lemak 2 kali sehari. Gunakan marjerin daripada jenis mono atau polyunsaturated.

Kurangkan santan dalam masakan. Gantikan dengan susu tanpa lemak. Elakkan mengambil kuah bersantan, hanya ambil isi sahaja gunakan periuk atau kuali non stick untuk menggoreng. Berhati-hati dengan lemak tersembunyi di dalam roti canai, pai dan sebagainya. Kurangkan penggunaan gula dan bahan manis.

Kurangkan garam dan makanan diawet dengan garam ( ikan masin, telur masin, tauchoo atau jeruk ) Amalkan cara memasak dengan mengukus atau merebus banyakkan mengambil makanan berserat seperti buahan, sayuran dan bijirin mil penuh seperti roti jagung, oats, cornflakes dan kekacang seperti gandum, barli, soya, dhal dll.

PANDUAN PEMAKANAN BAGI PESAKIT DARAH TINGGI

1. Makanan

-Kurangkan makanan bergaram. Jangan tambah garam ke dalam makanan yang sudah masak
-Kurangkan makanan laut seperti udang, kerang, ketam.
-Kurangkan makan daging kambing atau lembu. Boleh makan ayam yang dibuang kulit dan lemaknya.
-Putih telor boleh dimakan
-Kurangkan makanan bersantan dan berlemak seperti minyak sapi atau mentega

-Banyakkan makan rebusan dan kukusan

Pemakanan

1. Elakkan minum minuman manis sepeti sirap, milo, ovaltine, horlick dan oren. Elakkan minuman bergas dan air tin/ kotak.

Jantung Anda Bagaimana?

Walaupun ramai beranggapan sakit jantung tiada simptom, tetapi menurut Perunding Kardiologi Pusat Perubatan Universiti Malaya (PPUM), Dr Chong Wei Peng, ada lima tanda yang perlu diperhatikan. Katanya, FACES adalah cara mudah untuk mengenali tahap kesihatan diri. Walaupun ia bukan penunjuk tepat kepada masalah jantung, tetapi jika semua ciri umum ini ada pada kita, bermakna kita perlu bimbang terhadap tahap kesihatan diri.

Semua tanda mudah dikesan. Yang diperlukan hanya kepekaan terhadap perubahan diri kerana penjagaan kesihatan adalah tanggungjawab kita. Jika ada beberapa tanda, segera menjalani pemeriksaan kerana walaupun bukan disebabkan masalah jantung, ia mungkin menjadi petanda kepada penyakit lain.

Penyakit jantung tidak berlaku secara tiba-tiba. Ia mengambil masa bertahun-tahun untuk terbentuk secara tidak disedari. Sakit jantung adalah satu penyakit yang mempunyai spektrum luas kerana ia berkait dengan masalah lain seperti tekanan darah tinggi, aterotrombosis dan kegagalan jantung.

Namun, paling penting ialah mengawal tekanan darah kerana tekanan tinggi menandakan jantung terpaksa bekerja lebih kuat untuk mengepam darah ke seluruh badan. Apabila jantung dipaksa bekerja lebih kuat daripada sepatutnya, ia menjadi besar dan kurang cekap, akhirnya menyebabkan kegagalan jantung.

Perunding Kardiologi UMMC, Profesor Dr Haizal Haron Kamar, pula berkata penyakit jantung atau kardiovaskular adalah pembunuh utama penduduk di negara ini dan jumlah pesakit tidak pernah berkurang sejak dua dekad lalu. Berdasarkan kenyataan Pertubuhan Kesihatan Sedunia (WHO) pada 2004, sembilan peratus daripada kemasukan pesakit ke semua hospital adalah disebabkan masalah kegagalan jantung dan edaran darah.

Apabila disebut mengenai sakit jantung, rasanya semua orang tahu. Namun, kata Dr Haizal, masih ramai tidak memberi perhatian kepada penyakit ini, termasuk faktor risiko. Malah, masih ramai juga tidak tahu mengenai atherotrombosis yang menyebabkan 28 peratus kematian! Ia jauh lebih tinggi berbanding kanser dan penyakit berjangkit lain.

Masalah Tersembunyi
Aterotrombosis berlaku apabila darah membeku di atas plak (lapisan lemak di dinding salur darah) pecah. Darah beku ini akan membuat lapisan di atas plak tadi. Pembentukan darah beku secara berterusan boleh menghalang aliran darah. Apabila aliran darah tersekat, ia menyebabkan serangan jantung (jika berlaku pada salur darah ke jantung) atau strok (jika saluran darah ke otak terhalang).

Dr Haizal berkata, faktor ini menjadi masalah asas kepada strok iskemia (aliran darah yang tidak mencukupi ke otak), serangan jantung, serangan iskemia, sindrom koronari akut dan kerosakan pembuluh darah. Ini sebabnya setiap orang perlu mengamalkan gaya hidup sihat dan mengelak semua faktor risiko yang boleh menyebabkan penyakit jantung.
Hanya ada empat faktor risiko yang tidak boleh diubah iaitu jantina (lelaki), peningkatan umur, bangsa dan sejarah keluarga mendapat sakit jantung. Manakala faktor risiko seperti senarai di bawah boleh diubah dan dikawal iaitu:

  • Kencing manis.
  • Paras kolesterol tinggi.
  • Tekanan darah tinggi.
  • Kurang aktiviti fizikal dan bersenam.
  • Masalah berat badan berlebihan dan obesiti.
  • Pemakanan tidak seimbang.
  • Merokok.
Pensyarah dan Perunding Kardiologi PPUM, Dr Nik Halmey Nik Zainal Abidin, berkata gejala seperti hidung sering berdarah dan sakit kepala adalah petanda tekanan darah terlalu tinggi. Bagaimanapun, jika tekanan tidak terlalu tinggi (tetapi melebihi tahap normal iaitu kurang 140/85 mmHg) ia tidak mempunyai gejala."
Ini sebabnya ramai tidak sedar mereka sebenarnya menghidap tekanan darah tinggi. Jika tidak dikawal ia boleh menyebabkan kerosakan pada buah pinggang, mata dan otak. Penurunan sebanyak dua mmHg membawa banyak manfaat kepada pesakit kerana ia dapat mengurangkan tujuh peratus kematian.

"Kira-kira 24% rakyat Malaysia berusia 30 tahun ke atas mengalami masalah tekanan darah tinggi, tetapi hanya 18 peratus mendapatkan rawatan dan mengawal tekanan darah mereka. Manakala 13% mengambil ubat tetapi tekanan darah tidak terkawal dan 53% lagi tidak tahu mereka ada masalah ini," katanya.

Cara mudah untuk mengawal tekanan darah ialah menjaga pemakanan dengan mengurangkan pengambilan garam (hanya satu camca teh sehari) dan bersenam. Bagaimanapun, bukan mudah untuk menjaga pemakanan di negara yang mewah dengan sajian enak ini, tetapi semuanya perlu dilakukan demi kesejahteraan diri.

Some USEFUL information on CANCER

Every person has cancer cells in the body. These cancer cells do notshow up in the standard tests until they have multiplied to a fewbillion. When doctors tell cancer patients that there are no more cancercells in their bodies after treatment, it just means the tests areunable to detect the cancer cells because they have not reached thedetectable size.Cancer cells occur between 6 to more than 10 times in a person'slifetime.

When the person's immune system is strong the cancer cells will bedestroyed and prevented from multiplying and forming tumors.When a person has cancer it indicates the person has multiplenutritional deficiencies. These could be due to genetic, environmental,food and lifestyle factors. To vercome the multiple nutritional deficiencies, changing diet andincluding supplements will strengthen the immune system.

Chemotherapy involves poisoning the rapidly-growing cancer cells andalso destroys rapidly-growing healthy cells in the bone marrow,gastro-intestinal tract etc, and can cause organ damage, like liver,kidneys, heart, lungs etc.Radiation while destroying cancer cells also burns, scars and damageshealthy cells, tissues and organs.Initial treatment with chemotherapy and radiation will often reducetumor size. However prolonged use of chemotherapy and radiation do notresult in more tumor destruction.

When the body has too much toxic burden from chemotherapy and radiationthe immune system is either compromised or destroyed, hence the personcan succumb to various kinds of infections and complications.Chemotherapy and radiation can cause cancer cells to mutate and becomeresistant and difficult to destroy. By refraining from oreating less meat it frees more enzymes to attack the protein walls ofcancer cells and allows the body's killer cells to destroy the cancercells.

Some supplements build up the immune system (IP6, Flor-ssence, Essiac,anti-oxidants, vitamins, minerals, EFAs etc.) to enable the body's ownkiller cells to destroy cancer cells. Sugar substitutes like NutraSweet,Equal, Spoonful, etc are made with Aspartame and it is harmful. A betternatural substitute would be Manuka honey or molasses but only in verysmall amounts.

Table salt has a chemical added to make it white incolour. Better alternative is Bragg's aminos or sea salt.Milk causes the body to produce mucus, especially in thegastro-intestinal tract. Cancer feeds on mucus. By cutting off milk andsubstituting with unsweetened soya milk cancer cells are being starved. Cancer cells thrive in an acid environment. A meat-based diet is acidicand it is best to eat fish, and a little chicken rather than beef orpork.

Meat also contains livestock antibiotics, growth hormones andparasites, which are all harmful, especially to people with cancer.A diet made of 80% fresh vegetables and juice, whole grains, seeds, nutsand a little fruits help put the body into an alkaline environment.About 20% can be from cooked food including beans. Fresh vegetablejuices provide live enzymes that are easily absorbed and reach down tocellular levels within 15 minutes to nourish and enhance growth ofhealthy cells.

To obtain live enzymes for building healthy cells try anddrink fresh vegetable juice (most vegetables including bean sprouts) andeat some raw vegetables 2 or 3 times a day. Enzymes are destroyed attemperatures of 104 degrees F (40 degrees C).Avoid coffee, tea, and chocolate, which have high caffeine. Green tea isa better alternative and has cancer-fighting properties. Water-best todrink purified water, or filtered, to avoid known toxins and heavymetals in tap water. Distilled water is acidic, avoid it.

Vitamins Could Shorten Lifespan

According to UK media reports on 28 February, the study published in the journal of the American Medical Association (JAMA) showed that three types of supplements - vitamins A, E and beta-carotene-could increase the risk of early death. Research shows that vitamin supplements are a waste of money.

The British Heart Foundation recommends a good balanced diet over taking vitamin supplements.The Copenhagen University Hospital research team analyzed a lot of information. The analysis method is developed by Cochrane Collaboration, a renowned organization that specializes in medical analysis.

Vitamin supplement supporters believe that vitamins can act as antioxidants to prevent the damaging oxidizing effects of cholesterol etc on health, thereby helping heart disease and cancer prevention. This theory seems to make sense and is supported by initial tests. However, later tests reveal different results.The Copenhagen research team selected 68 randomized trials with 232,606 participants, including 47 low-bias trials with 180,938 participants.

They discovered that taking vitamin supplements, on the whole, increased death risk by 5%. Beta-carotene produced an approximate 7% increase risk, vitamin A, a 16% increase and vitamin E, a 4% increase. The risk of death was unchanged among vitamin C users. Certain trials showed that selenium, singly or combined with other vitamins, might lower death risk by 10%.

There is no evidence that vitamin C may increase longevity. We lack evidence to refute a potential negative effect of vitamin C on survival. Selenium tended to reduce mortality, but we need more research on this question, "stated Dr. Goran Bjelakovic, leader of the research team. In addition, beta-carotene might increase the risk of lung cancer among smokers.

Qn: What are the thumb rules for a layman to take care of his heart?
Ans:
1. Diet - Less of carbohydrate, more of protein, less oil.

Ans: No
Qn: It's still a grave shock to hear that some apparently healthy person gets a cardiac arrest. How do we understand it in perspective?
Ans: This is called silent attack; that is why we recommend everyone past the age of 30 to undergo routine health checkups.

Qn: Are heart diseases hereditary?
Ans: Yes

Qn: What are the ways in which the heart is stressed? What practices do you suggest to de-stress?

Qn: How do irregular eating habits affect the heart?
Ans: You tend to eat junk food when the habits are irregular and your body's enzyme release for digestion gets confused.

Qn: How can I control cholesterol content without using medicines?
Ans: Control diet, walk and eat walnut.

Qn: Can yoga prevent heart ailments?
Ans: Yoga helps.

Qn: Which is the best and worst food for the heart?

Ans: Help the person into a sleeping position, place an aspirin tablet under the tongue with a sorbitrate tablet if available, and rush him to a coronary care unit since the maximum casualty takes place within the first hour.

Qn: How do you differentiate between pain caused by a heart attack and that caused due to gastric trouble?
Ans: Extremely difficult without ECG .


Qn: What is the main cause of a steep increase in heart problems amongst youngsters? I see people of about 30-40 yrs of age having heart attacks and serious heart problems.
Ans: Increased awareness has increased incidents. Also, sedentary lifestyles, smoking, junk food, lack of exercise in a country where people are genetically three times more vulnerable for heart attacks than Europeans and Americans.


Qn: Is it possible for a person to have BP outside the normal range of 120/80 and yet be perfectly healthy?
Ans: Yes.


Qn: Marriages within close relatives can lead to heart problems for the child. Is it true?
Ans: Yes, co-sanguinity leads to congenital abnormalities and you may not have a software engineer as a child


Qn: Many of us have an irregular daily routine and many a times we have to stay late nights in office. Does this affect our heart? What precautions would you recommend?
Ans: When you are young, nature protects you against all these irregularities. However, as you grow older, respect the biological clock.


Qn: Will taking anti-hypertensive drugs cause some other complications (short / long term)?
Ans: Yes, most drugs have some side effects. However, modern anti-hypertensive drugs are extremely safe.

Qn: Will consuming more coffee/tea lead to heart attacks?
Ans: No.

Qn: Are asthma patients more prone to heart disease?
Ans: No.

Qn: How would you define junk food?
Ans: Fried food like Kentucky , McDonalds, samosas, and even masala dosas.

Qn: You mentioned that Indians are three times more vulnerable. What is the reason for this, as Europeans and Americans also eat a lot of junk food?
Ans: Every race is vulnerable to some disease and unfortunately, Indians are vulnerable for the most expensive disease.

Qn: Does consuming bananas help reduce hypertension?
Ans: No.

Qn: Can a person help himself during a heart attack (Because we see a lot of forwarded emails on this)?
Ans: Yes. Lie down comfortably and put an aspirin tablet of any description under the tongue and ask someone to take you to the nearest coronary care unit without any delay and do not wait for the ambulance since most of the time, the ambulance does not turn up.

Qn: Do, in any way, low white blood cells and low hemoglobin count lead to heart problems?
Ans: No. But it is ideal to have normal hemoglobin level to increase your exercise capacity.

Qn: Sometimes, due to the hectic schedule we are not able to exercise. So, does walking while doing daily chores at home or climbing the stairs in the house, work as a substitute for exercise?
Ans: Certainly. Avoid sitting continuously for more than half an hour and even the act of getting out of the chair and going to another chair and sitting helps a lot.


Qn: Is there a relation between heart problems and blood sugar?
Ans: Yes. A strong relationship since diabetics are more vulnerable to heart attacks than non-diabetics.


Qn: What are the things one needs to take care of after a heart operation?
Ans: Diet, exercise, drugs on time , Control cholesterol, BP, weight.

Qn: Are people working on night shifts more vulnerable to heart disease when compared to day shift workers?
Ans: No.


Qn: What are the modern anti-hypertensive drugs?
Ans: There are hundreds of drugs and your doctor will chose the right combination for your problem, but my suggestion is to avoid the drugs and go for natural ways of controlling blood pressure by walk, diet to reduce weight and changing attitudes towards lifestyles.

Qn: Does dispirin or similar headache pills increase the risk of heart attacks?
Ans: No.


Qn: Why is the rate of heart attacks more in men than in women?
Ans: Nature protects women till the age of 45.

Qn: How can one keep the heart in a good condition?
Ans: Eat a healthy diet, avoid junk food, exercise everyday, do not smoke and, go for health checkup s if you are past the age of 30 ( once in six months recommended) ...
